Q: I am transferring my child’s care from another practice. Are there things I need to do before the first visit at your practice?

A: Yes, before your child’s first visit we will need to have a copy of the records from your child’s previous healthcare provider. This is important for us to provide thorough and safe care for your child.

1. Please fill out a medical records release form and request they send it to our address (500 Holly Springs Rd. Suite 101 Holly Springs NC 27540) or fax it to 919-762-1530.

2. If you have a copy of your child’s immunization record, please bring it to the date of the appointment.

 

Q: Will I be charged for an school/sports form?

A: If the form is completed on the day of the exam, no. If you bring the form months after the exam, you will be charge $10 and you will have to wait at least 24 hours for the form to be completed.

 

Q: How long does it take a school/sports form to be completed?

A: We carry most of the official school and sports forms for the State of North Carolina School system. Forms will be generated automatically through the Electronic Health Records on the same day of the appointment. If you bring the forms later on, you will have to wait at least 24 hours for the form to be completed.

 

Q: What is the Patient Portal and how can I access it?

A: A patient portal is a secure online website through the Electronic Medical Record that gives you a convenient 24-hour access to your child's health information from anywhere with an Internet connection. Using a secure username and password that will be assigned on your first appointment, patients can view information such as:

• Recent doctor visits

• Medications

• Immunizations

• Allergies

• Lab results

• Check benefits and coverage
